Do I need to fill out paper work before my visit?
We ask that all patients register on the patient portal and complete the online patient forms. Every year you will be asked if anything has changed.
What should I bring to my appointment?
Please bring a list of all medications to your appointment.
Do I need to wear a face mask?
We do not require masks in our office however, If you would like the doctor to wear one please let the front desk know.
Can I order my contacts online?

Do you take my insurance?
We accept many insurance providers. We participate with, Advantica, Aetna Coventry, AlwaysCare, Anthem Blue Cross, Avesis, Carpenter's H&W Fund, Cigna, Davis Vision, Essence, Evercare, Eyemed, First Look, Healthlink, Humana, IBEW, March Vision, Medicare, Medicare Complete, Medicaid, Missouri Care, NVA, Opticare, Optum Health Vision, SVS, Tricare, UHC, VBA, VCP, VSP, Wellcare, we are a Crown Authorized Provider. Check with our staff if you have other types of coverage.
Can I have a copy of my prescription?

Can I use my frames for new lens? YES
The lab will ask you to sign a responsibility waiver.
Can I buy a pair of glasses or contacts with another doctors prescription?
Yes! However, We offer 50% off all designer frames if you see one of our doctors.
Do I have to have my eyes dilated?
No! Dr. McCullough and Dr. Harris recommend having your eyes dilated annually. However we offer an affordable alternative.
What age should my child have an eye exam?
We see patients starting at age 3.
How do I clean my Eyeglass lens?
For proper eyeglass cleaning, we recommend that you always clean your glasses while they are wet with an eyeglass cleaning solution or lukewarm water, and always using a microfiber cloth or a pure cotton cloth.
You can also make your own lens cleaning solution with 1 part rubbing alcohol and 3 parts water. This will help prevent scratching and damage to your lenses.
